Photo & Media Consent Form

This form must be completed by a parent or legal guardian.

1. Purpose of This Form

CGMax FFTP — Flag Football Tracker Pro allows coaches to associate a player photo with each athlete's profile within the app. This form authorizes the collection and use of your child's photograph for the purposes described below.

By signing this form, you provide informed consent as required under the Children's Online Privacy Protection Act (COPPA) and applicable state privacy laws.

2. How Photos Are Used

Your child's photo will only be used for:

Displaying the player's profile image within the CGMax FFTP coaching app on the coach's personal device
Appearing on digitally generated game scorecards and stat summaries viewable only by the coaching staff
If uploaded through this form, securely submitted via HTTPS to ITCC LLC's backend, stored in an access-controlled database, and a confirmation notification sent to the coaching staff

Photos will NOT be: shared on social media, used for advertising or marketing, sold to third parties, or made publicly accessible in any way. If you upload a photo through this form, it is stored in a secure, access-controlled database linked exclusively to the team's licensed coach — no other coaches, users, or third parties can access it.

3. Data Storage & Security

Player photos added directly through the app are stored in the web browser's local storage on the coach's personal device. Photos uploaded through this consent form are resized to a maximum of 800 pixels, then submitted via encrypted HTTPS to ITCC LLC's secure serverless backend and stored in a cloud database (Supabase) protected by Row Level Security — meaning only the licensed Pro coach linked to this team can access the photo. No other users, coaches, or third parties have access. An email notification is simultaneously sent to the coaching staff as a backup record of the consent submission.

The coach is responsible for maintaining the physical security of their device. Photos can be deleted at any time by removing the player from the roster within the app, by clearing the browser's local storage, or by contacting cgmaxfftp@itcc.llc to request permanent deletion from the database.

4. Children's Privacy (COPPA)

CGMax FFTP does not knowingly collect personal information from children under 13 without verifiable parental consent. This form serves as that verifiable consent. By signing below, you confirm that:

You are the parent or legal guardian of the named child
You understand how the photo will be used and stored
You have the legal authority to provide this consent
The child is a participant on the team identified in this form

5. Your Rights

You may withdraw this consent at any time by notifying the coach verbally or in writing. Upon withdrawal, the coach will promptly delete your child's photo from the app. You also have the right to:

Request to see the photo currently stored in the app
Request immediate deletion of the photo at any time
Decline to sign this form — your child may still fully participate on the team without a profile photo

This consent is valid for the current athletic season only and must be renewed for subsequent seasons.

7. Player Photo Optional

Upload a recent photo of your child (e.g. their league photo from last season). This photo will be used as their player profile image within the coaching app only.

Photos are resized to 800px max and stored securely. You may skip this step — the coach can also take a photo during practice.

8. Acknowledgment & Signature

I understand I may withdraw this consent at any time by notifying the coach, and that my child may participate without a photo.
